Fresenius Kabi we aim to provide a smooth running of the service that will deliver feed requirements directly into the patients home

When setting up a patient on the Fresenius Kabi Homecare system there are a series of easy steps to follow:

• Dietitian completes the patient activation form and sends to Fresenius Kabi Homecare
• Fresenius Kabi register the patient on the Homecare System and forward the adult or paediatric patient welcome information pack
• Dietitian contacts the Homecare Nurse Advisor to arrange training
• Homecare Nurse Advisor trains the patient / carer on the feeding system
• Dietitian contacts the GP to request a prescription
• Patient Care Co-ordinator calls the patient / carer to arrange first delivery
• The Patient Care Co-ordinator follows up with a courtesy call
• Patient Care Co-ordinator liases with the patient for further supplies
